Eligibility
Benefits-eligible employees, their spouses and dependent children
Coverage
Employees may elect coverage up to five times their annual wage with a maximum of $750,000 for self and $500,000 for spouse. Coverage amounts are available for dependent children in two thousand dollar increments up to a maximum of $10,000.
Cost
The employee is responsible for the full cost. Premiums for employee and spouse coverage are based on age and adjusted annually effective with the January premium, which is deducted from the December paychecks.
