Personal Cancer Plan
An optional cancer indemnity plan is available to you and your dependents. The AFLAC plan offers cancer coverage plus three riders. The Cancer Indemnity Plan pays a $3000 one time only Initial Treatment Benefit when a covered person receives Radiation Therapy, Injected Chemotherapy, or
Oral Chemotherapy. Other benefits include: Indirect/Additional Therapy, Surgical Treatment, Hospitalization, Continuing Care, and Ambulance/Transportation/Lodging. An Optional Initial Diagnosis Benefit Rider may be added to the Cancer Indemnity Plan. This rider pays, in addition to other benefits, $2500 for a covered
employee/spouse and $5000 for a covered dependent child when diagnosed with Internal Cancer or an Associated Cancerous Condition. Enrollment in the Cancer
Indemnity Plan is required. An Optional Cancer Screening and Annual Care Benefit Rider is available also. This rider pays $75 per calendar year for each covered person in addition to other
benefits. Enrollment in the Cancer Indemnity Plan is required. You may purchase a Specified Disease Rider if you have the Cancer Indemnity Plan. This rider covers specific illnesses.
Coverage can be purchased within 31 days of your first day of employment, or during open enrollment each fall. The full cost of this optional coverage is paid by the employee. An HRM Benefits Counselor can provide rates and additional coverage details.
